Each wellness appointment is an opportunity for important communication between the nurse and the client. The wellness participant has the chance verbalize any questions or concerns, while the nurse listens for any educational deficits that need to be addressed. Our nurse evaluators are all experienced in patient education, so at this point some good teaching can take place targeting areas of concern.
As each wellness visit comes to an end, the discussion turns toward a strategy geared toward meeting the goals for the next evaluation. Access to relevant resources such as community agencies, websites, phone applications, and other health professionals such as dietitians is verified and reviewed.
The client also has access to our Wellness Portal that provides education modules that are recommended based on current assessments, as well as information on any health topic that may be of interest to them.
